You can't mori all 4 with a piece. You need tuft + full tombstone. You cannot combo tuft with piece, after the mori, you will be set back to tier 2. You can only get the achievement by running both tombstone and tuft, otherwise there is not enough evil to stalk. I used judith's and tuft when I did it.
